Error creating the add-in from the DLL "*.dll" Cause: The operation was cancelled by the user

Has anyone seen this?

AddAddIns API call from a standalone.

I feel this API call is only meant to be used from an add-in to install another one. Not sure how it was cancelled. It is giving me vibes from the EdmCmd.poCancel
